Required Forms
Once you have contacted UTAP (Dolores Groves at 512-232-1715) and your registration fee of $35 per person over the age of 12 has been received, you will be given a confirmation number and be asked to complete the forms listed below.
All persons attending the training are required, by the University of Texas Office of Legal Affairs, to print out the following documents as they pertain to each individual. Please fill out and sign the documents prior to arriving at the training site. You will be asked for all completed documents at check-in, along with your confirmation number.

| Talent Release Form | This document must be filled out by every person who is attending the training. |
| Minor Release Form | This document must be filled out for each person under age 18 who will be attending the training. |
| Non-Student Adult Release Form | This document must be filled out by each person over age 18 who is attending the training. |
| Minor Medical Release Form | This document must be filled out for each person under age 18 who will be attending the training. |
| Adult Medical Form | This document must be filled out by each person over age 18 who is attending the training. |
| Student Adult Release Form | This document is only for people who will be attending the training and are currently enrolled at the University of Texas as a student at the time of the training. |
