Anecdotal record forms are used for general observations that are kept in the student's file.

Formative Assessment
The Formative Assessment is used to evaluate students in the Intern semesters, as well as mid-semester in the Student Teacher semester. The "short" form may be used for the Intern I or Intern II semesters. The "long" form should be used for the Intern II semester, the Intern III (SED) semester, and mid-semester for the Student Teacher semester.

Summative Assessment
The Summative Assessment is used to evaluate students at the end of the Student Teacher semester, and serves as the final evaluation for students completing the PDS. The official Summative Assessment is completed in the Online Evaluation System by the facilitator and cooperating teacher. The paper-based Summative Assessment may be downloaded below for students to complete a self-assessment.
