Summative Assessment Instructions

APPRENTICE TEACHING

The Summative Assessment is completed at the end of the apprentice teaching semester only.

Guidelines:

  • Summative Assessments are completed by the facilitator and the cooperating teacher each on a separate form, and are then discussed in a three-way meeting with the UT student. The UT student will read the Summative Assessment and ask for clarification, if needed.
  • The facilitator and cooperating teacher will sign their respective assessments and the student will sign both copies.
  • Both copies will comprise the final legal documentation.
  • All Summative Assessments must be signed by all participants (the facilitator, the cooperating teacher, and the student). This does not mean that the apprentice teacher agrees with the assessment; only that she or he has read it.
  • Original copies of the Summative Assessments are due to the Education Services office (SZB 294) no later than the Wednesday during the last week of classes.
  • Fill in the complete names of the UT student, cooperating teacher, and facilitator - no nicknames, please.
  • Fill in the date of the assessment.
  • Fill in the name of the school in which the practicum is being held - please use the full name of the school (e.g., Barrington Elementary).
  • Fill in the district in which the practicum is being held (e.g., Austin ISD).
  • Fill in the grade level for elementary practicum or the grade and subject area for secondary practicum.
  • Rate each observable behavior, if there was no opportunity to observe a specific behavior then you may rate that one as “NA."
  • A rating of “0” or “1” in any of the areas marked with an “*” may result in a grade of "F" in the apprentice teaching practicum.
  • Rate each "Component" as a whole using the lettered scale (U, EC, C, AC).
  • It is very important to cite specific examples of the observed behaviors as Supporting Evidence at the end of each of the four clusters.
  • It is the responsibility of the facilitator to collect all signed Summative Assessments and show them to the program coordinator for review before turning them into the Education Services office.
  • After review, the coordinator will initial each assessment in the signatures box.
  • It is important that the Summative Assessments are legible and are presented in a professional manner, as each Summative Assessment is submitted as legal documentation of their performance.
