To Register

You may register for all classes by mail, or in person at the UT Aquatic Safety Program office located in Bellmont Hall room 222. The office is open from 8 a.m. - 4:45 p.m. Monday-Friday.

Consent and Medical Release Forms:

These forms will be mailed once registration is received. Completed forms must be received before the first day of class (Download required forms: Adult Participant, Minor Participant).

Notice of Privacy Practices. 

***Please note that one form requires a physician's signature stating that the participant has had a physical within the past 12 months and is free to participate in class.

Registrants will not be allowed to participate in class until the completed forms are returned to the UTASP office.

Refunds

Due to enrollment demands in our classes, if you withdraw PRIOR to the first day of class you will receive a refund minus a 20% processing and handling fee. Allow 6-8 weeks for refund checks.

There are no refunds if you withdraw after the first day of class.
