Medical and Consent Release Forms:

**Registrants will not be allowed to participate in class until all completed forms are returned to the Summer Sports School office.**

Forms will also be emailed once registration is received. Completed forms must be received before the first day of class.

Forms must be returned to the Summer Sports School office. Please do NOT bring them to class on the first day.

**Please note that there is a Physician's Release Form, which must be signed by a physician stating that the participant is able to participate and has had a physical within the past 12 months. These forms may be kept on file for up to one year.**
