Registration Forms

You will be required to turn in one or more of the following forms during registration each semester. All forms can be downloaded from this web page or from the individual registration topic (Advising, Add/Drop, etc.) web pages. Click on any of the links to the left, under the Related Links header, in order to access the web pages for each registration topic. You will receive more detailed information about each registration topic from these web pages. Click on the appropriate icon below to download a PDF formatted copy of each registration topic form.

Remove your Advising Bar (All students, except doctoral candidates, must do this every semester)

Add or Drop a course OR Change the grade status of a course

Register for an Individual Instruction Course

Register for two or more courses that conflict in time OR select CR/NC for the grade status of two or more of the same Individual Instruction courses
