a. If the camper is a minor (under the age of 18), please complete the forms from
the link "for parents of minors (under 18 years of age) [PDF]"b. If the camper is 18 years old or older, please complete the forms from the link "For campers 18 years of age and older [PDF]"
c. The packet says to mail the completed forms to us, please do NOT mail them to us. PLEASE BRING ALL FORMS WITH YOU TO REGISTRATION.
d. For the forms indicated as "sign and return," please complete these forms and BRING THEM WITH YOU TO REGISTRATION - please do NOT mail them to us ahead of time.
e. For the form indicated as "have physician sign and return," please have your child's physician complete the form and BRING IT WITH YOU TO REGISTRATION - please do NOT mail them to us ahead of time.
